Sublobar resection in high-risk patients for lobectomy: current and future strategy

نویسندگان

چکیده

Surgical resection by lobectomy is the gold standard of therapy for early stage non-small cell lung cancer. However, not all patients are medically fit to undergo surgery. In considered high-risk lobectomy, alternative strategies have been developed including radiofrequency ablation, cryoablation, microwave stereotactic radiation therapy, wedge resection, and segmentectomy. This work reviews definition high-risk, outcomes that associated with each treatment technique. Some technical points regarding versus segmentectomy noted. Future directions discussed in context at lobectomy.

Sublobar resection for lung cancer.

Sublobar resection for small lung cancers has been debated frequently and is still a controversial issue. The only randomised trial comparing lobectomy with sublobar resections found a significantly higher recurrence rate for the latter, but failed to show significant differences in survival, although survival was better for the lobectomy group. One meta-analysis and several nonrandomised compa...

Sublobar resection versus lobectomy in solid-type, clinical stage IA, non-small cell lung cancer

BACKGROUND Recent studies have demonstrated that sublobar resection is not inferior to lobectomy for peripheral early lung cancer with ground-glass opacification. However, the effect of sublobar resection on solid-type early lung cancer is controversial. The aim of this study was to compare clinical outcomes of patients who have undergone sublobar resection or lobectomy for solid-type, early-st...

Lobectomy versus Sublobar Resection in Non-Lepidic Small-Sized Non-Small Cell Lung Cancer

Background Recently, many surgeons have chosen sublobar resection for the curative treatment of lung tumors with ground-glass opacity, which is a hallmark of lepidic lung cancer. The purpose of this study was to evaluate the oncological results of sublobar resection for non-lepidic lung cancer in comparison with lobectomy. Methods We conducted a retrospective chart review of 328 patients with...

Sleeve lobectomy: current indications and future directions.

Sleeve lobectomy was initially conceived as an alternative to pneumonectomy for patients with low-grade, centrally located lesions and limited cardiopulmonary reserve. Over the last several decades, advances in patient selection criteria and surgical techniques have allowed sleeve lobectomy to evolve from a compromise to pneumonectomy to first line intervention for centrally located lesions of ...

Treatment of stage I lung cancer in high-risk and inoperable patients: SBRT vs. RFA vs. sublobar resection.

Although surgical resection for early stage lung cancer is the mainstay of treatment, many patients are inoperable at the time of presentation due to either disseminated disease or medical comorbidities (1). Novel strategies are currently being developed to treat early-stage non-small cell lung cancer (NSCLC) in this expanding population of highrisk and inoperable patients. Stereotactic body ra...
